Efficacy of bi-monthly hepatic arterial infusion chemotherapy for advanced hepatocellular carcinoma in patients with decompensated cirrhosis

The efficacy of bi-monthly hepatic arterial infusion chemotherapy (B-HAIC) was not inferior to that of Sorafenib chemotherapy for the treatment of advanced hepatocellular carcinoma (aHCC) in patients with compensated cirrhosis. In this study, we demonstrate the efficacy of B-HAIC in patients with decompensated cirrhosis.



The efficacy rate and the disease control rate in the Child–Pugh B group (21% and 71%, respectively) were not significantly impaired compared with the rates in the Child–Pugh A group (38% and 67%, respectively). The median survival time and the survival rate at 12 months of patients in the Child–Pugh B group were 422 days and 58.3%, respectively, whereas those in the Child–Pugh A group were 567 days and 70.8%, respectively. Importantly, the hepatic functional reserve of patients in each group did not worsen during the treatment period. Furthermore, the occurrence rate of serious adverse events leading to discontinuation of anti-tumour treatment in both groups was quite limited despite the preserved hepatic functional reserve in these patients.

Cancer prevention drug also reduces virulence of H. pylori bacterium

The investigators found that in addition to its known ability to block the production of cell growth compounds, the drug DFMO (difluoromethylornithine) acts directly on the bacterium Helicobacter pylori to reduce its virulence. H. pylori infection is the primary cause of gastric cancer.

H. pylori infects the stomachs of half of the human population, but only about 1 percent of infected individuals develop stomach cancer. Although it's possible to treat the infection to prevent stomach cancer, it's not clear whom to treat. Plus, the bug may be conferring beneficial effects -- esophageal reflux diseases, asthma and other allergic disorders occur more frequently in people who are not infected with H. pylori.

Dendritic Spine Pathology in a Schizophrenia-related Neurodevelopmental Model

Schizophrenia is a neurodevelopmental disorder identified by psychotic episodes, as well as cognitive deficits. There are reports of multiple alterations in the brain in schizophrenia, such as fewer dendritic spines with shape abnormalities in cortical and subcortical areas. A correlation has been established between shape and function in dendritic spines; and as the main site of excitatory synapses, the dendritic spine pathology represents a potential target for novel therapeutic tools in schizophrenia.

Neonatal ventral hippocampus lesion (nVHL) in the rat has been extensively used to examine some of the pathophysiological mechanisms of schizophrenia since it mimics several behaviors, as well as abnormalities in morphological corticolimbic neurons. Thus, in this report, we determined the effects of the nVHL on locomotor behavior and memory in postpubertal rats.

pretreatment neutrophil-to-lymphocyte ratio in the survival of primary parotid cancer patients

A total of 249 candidates were studied to estimate the ability of pretreatment neutrophil-to-lymphocyte ratio (NLR) to predict survival in patients with parotid cancer. They reported a mean NLR of 2.48, with a range from 1.5 to 6.1; intraparotid node metastasis in 18.1% of cases. They found a significant association of NLR with tumor stage, disease stage, and disease grade. They recorded a 10 -year disease-specific survival (DSS) rate of 62%; 73 patients died due to the disease. They found a 10 -year DSS rate of 68% and 58% in the cases with an NLR<2.48 and an NLR≥2.48, respectively, with a significant difference. They observed the NLR as an independent prognostic factor for DSS in the Cox model analysis.

New Blood Test which can spare cancer patients Chemotheraphy

The blood test, also called a “liquid biopsy” is being trialled all across Australia and New Zealand at more than 40 hospitals, and is showing promising results.Chemotherapy, a common treatment for a number of cancers, is currently used post surgery to remove any “residue” of the cancer that may be left behind.While some people are completely cancer free after having the tumour surgically removed, others are at risk of a recurrence. Currently patients are assessed using results from pathology testing of the tumour and evaluation of other risk factors.This means chemotherapy is prescribed for patients who are considered high risk, without knowing whether it’s necessary, to ensure the cancer does not return.So there are patients who are being administered anti-cancer drugs when they may not need them.

Genomics could guide treatment for pancreatic cancer patients

One reason why pancreatic cancer is so deadly is that the majority of patients often are identified late in their disease course and frequently present with inoperable tumors at the time of diagnosis. For some of these patients, it may be possible to shrink the tumor with existing chemotherapy drugs, but in a disease where 75 percent of patients die within a year of diagnosis, time is of the essence, and unfortunately, there's no way to know in advance which patients will respond to which drugs.

Micro-Platform Reveals Natural Behaviour Of Cancer Cells

The observation shed light on the mechanisms behind well-known properties of pancreatic cancer, one of the most lethal malignant tumours, and may lead to the identification of new treatment targets.The researchers created a new cell culture substrate from a coated glass slide with etched islands of 30 Β΅m diameter. For healthy cells, this is just enough space for one or two to attach.

 But when the researchers seeded them with pancreatic cancer cells (and other tumour cells) and incubated them overnight, the cells self-organised into micro-tumours (pictured) that could move in a concerted way, as if it were one organism. Precursors to this turned out to be papillary structures that accommodate four or more cells by cell-in-cell invasion. This process, called entosis, is so far known only as a step in cell degradation, but in this study the incorporated cells remained alive and the incorporation was reversible.

Solid stress in brain tumours causes neuronal loss and neurological dysfunction and can be reversed by lithium

The compression of brain tissue by a tumour mass is believed to be a major cause of the clinical symptoms seen in patients with brain cancer. However, the biological consequences of these physical stresses on brain tissue are unknown. Here, via imaging studies in patients and by using mouse models of human brain tumours, we show that a subgroup of primary and metastatic brain tumours, classified as nodular on the basis of their growth pattern, exert solid stress on the surrounding brain tissue, causing a decrease in local vascular perfusion as well as neuronal death and impaired function. We demonstrate a causal link between solid stress and neurological dysfunction by applying and removing cerebral compression, which respectively mimic the mechanics of tumour growth and of surgical resection. We also show that, in mice, treatment with lithium reduces solid-stress-induced neuronal death and improves motor coordination. Our findings indicate that brain-tumour-generated solid stress impairs neurological function in patients, and that lithium as a therapeutic intervention could counter these effects.

Adrenal Pathology in the Adult: A Urological Pathologist’s Perspective

Adrenal gland diagnostics can pose significant challenges. In most academic and community practice settings, adrenal gland resections are encountered less frequently than other endocrine or genitourinary specimens, leading to less familiarity with evolving classifications and criteria. The unique dichotomy between cortical and medullary lesions reflects the developmental evolution of these functionally independent components. Adrenal cortical lesions at resection include hyperplasia, adenoma, and carcinoma, with some cases straddling the boundary between these distinct clinical classifications. The lack of immunohistochemical or molecular markers to definitively categorize these intermediate lesions enhances the diagnostic challenge.

 In addition, modified terminology for oncocytic and myxoid cortical lesions has been proposed. Medullary lesions are somewhat easier to categorize; however, the prediction of aggressive behavior in pheochromocytomas remains a challenge due to a lack of reliable prognostic biomarkers. Recent work by the Cancer Genome Atlas Project and other research groups has identified a limited subset of molecular and signaling pathway alterations in these 2 major neoplastic categories. Ongoing research to better define prognostic and predictive biomarkers in cortical and medullary lesions has the potential to enhance both pathologic diagnosis and patient therapy.

Cutaneous Desmoplastic Melanoma

Desmoplastic melanoma (DM) is a fibrosing variant of spindle cell melanoma. It most often presents as an indurated lesion in chronically sun-damaged skin. Due to the lack of characteristic clinical features, early detection is uncommon. At the time of excision, the tumors usually extend into the reticular dermis or deeper. DM is prone to misdiagnosis. It may simulate histologically sclerosing melanocytic nevi as well as various benign and malignant nonmelanocytic lesions. There is significant morphologic variability among tumors classified as DM. Desmoplasia may be prominent throughout the entire tumor (“pure” DM) or represent a portion of an otherwise nondesmoplastic melanoma.

 Some tumors show neuroma-like features with prominent nerve involvement, in which case the term “desmoplastic neurotropic melanoma” is used. Immunophenotypically, DMs are usually strongly and homogeneously positive for S-100 protein but are often negative or only focally positive for melanocyte differentiation antigens such as tyrosinase, gp100, Melan-A, and microphthalmia transcription factor. DM differs from conventional melanoma in its clinical course. It is associated with a higher tendency for local recurrence, but metastases to regional lymph nodes are less common. Evidence is also emerging that for patients with thick melanomas, the presence of a paucicellular fibrosing tumor histology (pure DM) is a favorable prognostic factor for survival.

The conditions included are autosomal-dominant polycystic kidney disease, autosomal-recessive polycystic kidney disease, unilateral renal cystic disease (localized cystic disease), renal simple cysts, multicystic dysplastic kidney, pluricystic kidney of the multiple malformation syndromes, juvenile nephronophthisis and medullary cystic disease, medullary sponge kidney, primary glomerulocystic kidney disease, and glomerulocystic kidney associated with several systemic disorders mainly of genetic or chromosomal etiology, cystic kidney in tuberous sclerosis, and in von Hippel-Lindau syndrome, cystic nephroma, cystic variant of congenital mesoblastic nephroma, mixed epithelial stromal tumor of the kidney, renal lymphangioma, pyelocalyceal cyst, peripylic cyst and perinephric pseudocyst, acquired renal cystic disease of long-term dialysis, and cystic renal cell carcinoma and sarcoma. Whereas the gross and histologic appearance of some of these conditions may be diagnostic, clinical and sometimes molecular studies may be necessary to define other types.

The Pathology of Acute Liver Failure

Acute liver failure (ALF) is a rare and severe liver disease that usually develops in 8 weeks or less in individuals without preexisting liver disease. Its chief causes worldwide are hepatitis virus infections  and drug hepatotoxicity . Massive hepatic necrosis is often seen in liver specimens in ALF and features marked loss of hepatocytes, variable degrees of inflammation, and a stereotypic proliferation of bile ductular structures (neocholangioles) derived from activated periportal hepatic progenitor cells. This paper reviews the liver pathology in ALF, including forms of zonal necrosis and their etiologies.

Role of Human Papillomavirus in Vulvar Cancer

Human papillomavirus (HPV) is involved in one of the at least 2 pathways leading to vulvar squamous cell carcinoma (VSCC). Inactivation of p53 and retinoblastoma by the viral products E6 and E7 is involved in malignant transformation. The percentage of HPV-positive VSCCs ranges from 18% to 75%, depending on the geographical area. HPV-associated tumors affect relatively young women and arise from high-grade intraepithelial lesions, identical to other HPV-associated premalignant lesions of the anogenital tract. HPV-independent tumors tend to affect older women and usually arise in a background of inflammatory skin disorders and a subtle variant of in situ lesion called differentiated vulvar intraepithelial neoplasia.

HPV-positive tumors tend to be of basaloid or warty types, whereas HPV-independent tumors tend to be of keratinizing type, but there is frequent overlap between histologic types. There is no conclusive evidence yet on the best strategy in terms of determining HPV attribution. HPV DNA detection is generally considered the gold standard although there is some concern about misclassification when using this technique alone. p16 immunostaining has shown to be an excellent surrogate marker of HPV infection. Positive results for both techniques are considered the best evidence for HPV-association. The prognostic role of HPV in VSCC is still contradictory, but increasing evidence suggests that HPV-associated tumors are less aggressive. Currently, there are no differences in treatment between HPV-associated and HPV-independent VSCC, but novel immunological strategies based on anti-HPV antigens are being evaluated in clinical trials.
